Espasmos Acomodativos: Revisión Bibliográfica sobre Prevalencia, Etiología y Tratamiento

Resumen

Relevancia: El espasmo acomodativo es una causa infradiagnosticada de pseudomiopía en niños y jóvenes, asociada a demandas intensas de visión próxima. La heterogeneidad diagnóstica y terapéutica dificulta su abordaje clínico, por lo que resulta necesaria una síntesis actualizada de la evidencia.

Resumen: El espasmo acomodativo es un trastorno poco frecuente caracterizado por una contracción involuntaria y sostenida del músculo ciliar que produce pseudomiopía y síntomas visuales fluctuantes. Se realizó una revisión bibliográfica estructurada en PubMed y Embase siguiendo las recomendaciones Preferred Reporting Items for Systematic Reviews and Meta‑Analyses 2020, incluyendo estudios originales y casos clínicos publicados entre 1976 y 2024 relacionados con prevalencia, etiología y tratamiento. Se seleccionaron 46 artículos, de los cuales 22 correspondieron a estudios originales y 24 a casos clínicos. La prevalencia descrita osciló entre el 0,3 % y el 5,4 %, con mayor frecuencia en población escolar sometida a elevadas demandas de visión próxima. Las causas identificadas fueron principalmente funcionales, psicógenas y orgánicas, frecuentemente con presentación multifactorial. Los tratamientos más utilizados combinaron cicloplejía farmacológica, lentes positivas y terapia visual, mostrando mejoría clínica en la mayoría de los casos descritos. La refracción ciclopléjica continúa siendo fundamental para diferenciar pseudomiopía y miopía verdadera. El abordaje multidisciplinar, junto con medidas de higiene visual y seguimiento clínico individualizado, parece mejorar la evolución funcional y reducir las recurrencias. La limitada calidad metodológica y la heterogeneidad de los estudios incluidos evidencian la necesidad de investigaciones prospectivas con criterios diagnósticos estandarizados y seguimiento prolongado.
